New Patient Registration Form

All new GP patients of The Village GP Black Rock must complete a New Patient Registration Form, even if they have seen one of our GPs before at another clinic. 

You can:

• do this electronically – your SMS and/or email appointment reminder will have a link to an electronic form, or you can click on the button below; or
• arrive 10 minutes early to complete on the practice iPad or a paper form in our reception area.
  If you are new to the area, you can also complete a New Patient Registration Form to pre-register with us.
