What is a Heart Health Check?

A Healthy Heart Check is a Medicarefunded cardiovascular disease (CVD) risk
assessment for people who do not have known heart disease. It assesses your risk
of having a heart attack or stroke and supports early prevention and ongoing heart
health management with your GP.

Who is it for?

  • A Heart Health Check is for people without known cardiovascular disease who meet
    agebased eligibility criteria, including:
    Adults aged 45–79 years
  • Aboriginal and Torres Strait Islander people aged 30–79 years
  • People aged 35–79 years with diabetes

Your GP will confirm eligibility and whether the assessment is appropriate for you.

What is involved?

  • A Heart Health Check is completed by your general practice team and may include:
     Review of your medical and family history
  • Measurement of blood pressure
  • Review of cholesterol and diabetes status, including blood tests if required
     Use of the Aus CVD Risk Calculator to estimate your risk
  • Discussion about lifestyle factors and ways to reduce risk
  • Development of a personalised management plan

The assessment may involve both a practice nurse and your GP, allowing adequate
time for assessment, discussion, and planning.

If you are concerned about your risk of heart disease, we recommend calling
reception and booking a Healthy Heart Check to allow adequate time for a
comprehensive cardiovascular risk assessment.

Some patients may be eligible for a Medicare-funded health assessment. Your GP can confirm your eligibility at the time of booking. 

The assessment is completed over two appointments

  • An initial appointment with our practice nurse (30-45 minutes) 
  • Followed by a GP appointment to review results and discuss next steps (20-30 minutes)
